Executive Summary
The rise of antibiotic-resistant bacteria is a major threat to public health and the economy, affecting both human and animal health. In the United States alone, millions of people acquire antibiotic-resistant infections each year, resulting in thousands of deaths. To address this issue, the U.S. Government developed a National Action Plan (NAP) for Combating Antibiotic-Resistant Bacteria (CARB) in 2015, with the goal of detecting, preventing, and controlling antibiotic-resistant infections over a five-year period. In 2018, the PACCARB was tasked with providing recommendations for the next iteration of the NAP from 2020 to 2025. The report highlights the progress made since the release of the NAP and identifies priorities for the future. Key achievements include promoting antibiotic stewardship in healthcare settings, enhancing antibiotic stewardship in veterinary settings, improving coordination and surveillance efforts, incentivizing the development of new products, and enhancing international collaboration and capacities. However, challenges remain such as the need for novel models to incentivize research and development, achieving AMR surveillance through a One Health approach, addressing fungal infections caused by resistant pathogens, and securing sustained funding and organizational support. The report recommends priorities such as research, implementation, and measurement of interventions, incentives for uptake of stewardship programs, data management and interoperability, funding state and local AMR programs, communication and awareness, U.S. global leadership, environmental research, and addressing fungal infections. Overall, the report emphasizes the importance of sustained efforts and collaboration to combat antibiotic resistance and reduce its impact on public health.

This report was compiled by the National Action Plan for Combating Antibiotic Resistant Bacteria Working Group, and was voted on and approved by the full council at the 13th PACCARB Public Meeting on July 10, 2019.
