Summary

According to the American Academy of Pediatrics, the medical home model of care “is a family-centered partnership within a community-based system that provides uninterrupted care with appropriate payment to support and sustain optimal health outcomes.” Many children in the United States don’t get care in a medical home. Evidence shows that children who do are more likely to have preventive health care visits and better health outcomes.
