You’ll also learn about a nonprofit community benefit organization working to increase access to primary care providers (PCPs) for vulnerable populations.

PCPs play an important role in protecting the health and safety of the communities they serve. By working in the context of family and community, they can provide integrated services and develop meaningful, sustained relationships with patients. Having a usual PCP is associated with greater patient trust in the provider, effective patient-provider communication, and increased likelihood that patients will receive appropriate care.

About Healthy People 2020 Leading Health Indicators

The Leading Health Indicators (LHIs) represent a smaller set of Healthy People 2020 objectives selected to communicate high-priority health issues and actions to help address them. LHIs are used to assess the health of the Nation, facilitate collaboration across sectors, and motivate action to improve the health of the U.S. population.
